1. How to Request Your Health Records

You have the right to access and obtain copies of your health records. To request your records:

  • Submit a written, signed, and dated request to our office

  • Include your full name, date of birth, and contact information

  • Specify the records you need (e.g., dates of service, type of records)

  • Allow up to 30 days for us to respond to your request

  • Note that a reasonable, cost-based fee may apply for copying and processing
