1. How to Request Your Health Records
You have the right to access and obtain copies of your health records. To request your records:
Submit a written, signed, and dated request to our office
Include your full name, date of birth, and contact information
Specify the records you need (e.g., dates of service, type of records)
Allow up to 30 days for us to respond to your request
Note that a reasonable, cost-based fee may apply for copying and processing
